The following terminology applies to these Terms and Conditions, Privacy Statement and Disclaimer Notice and any or all Agreements: “Client”, “You” and “Your” refers to you, the person accessing this website and accepting the Company’s terms and conditions. “The Company”, “Ourselves”, “We”, “Our” and “Us”, refers to our Company. “Party”, “Parties”, or “Us”, refers to both the Client and ourselves, or either the Client or ourselves. All terms refer to the offer, acceptance and consideration of payment necessary to undertake the process of our assistance to the Client in the most appropriate manner, whether by formal meetings of a fixed duration, or any other means, for the express purpose of meeting the Client’s needs in respect of provision of the Company’s stated services/products, in accordance with and subject to, prevailing law of . Any use of the above terminology or other words in the singular, plural, capitalisation and/or he/she or they, are taken as interchangeable and therefore as referring to same.
Cancellation and Refund Policy
Once the reservation is made, other customers will be blocked from that reserved time slot, and we will have to plan and arrange our personnel and resources for the reservation. Therefore, reservations and payments cannot be cancelled or refunded under most circumstances. An exception to this rule is force majeure or act of God prevents the testing services to occur, then, under these circumstances, the reservations and payments can be cancelled.
Please note, in any event that the reservations and payments are cancelled, we will charge a 15% processing fee, to cover the cost of our personnel and transaction cost.
Please note, by submitting the reservation and payment, you certify that you have read, understood, and agreed to this Cancellation and Refund Policy.
Timeline for Issuing Report
All the timelines described in the service descriptions are strictly estimates only, and cannot be guaranteed. Actual timeline may vary significantly due to volume, technical, and other unforeseeable reasons. If you do not agree to the test's description about the time schedule or if you do not agree to this Terms and Conditions, please leave this website and please do not submit the reservation and payment.

Time for the Tests
Not all tests can be scheduled or rescheduled at specific desired days and hours. Please read and agree to each test's description about the time schedule before you submit the reservation and payment. Actual time schedule may vary significantly due to regulatory, volume, technical, and other unforeseeable reasons. If you do not agree to the test's description about the time schedule or if you do not agree to this Terms and Conditions, please leave this website and please do not submit the reservation and payment.
Please note, by submitting the reservation and payment, you certify that you have read, understood, and agreed to this term on Time for the Tests.
Informed Consent for COVID-19 Testing
Please note, by submitting the reservation and payment, you certify that you have read, understood, agreed to, and electronically signed the following Informed Consent for COVID-19 Testing.
1. Authorization and Consent for Covid-19 Diagnostic Testing: I voluntarily consent and authorize Apostle Inc, Apostle Diagnostics to conduct collection, testing, and analysis for the purposes of a COVID-19 diagnostic test. I acknowledge and understand that my COVID-19 diagnostic test will require the collection of an appropriate sample by my healthcare provider through a nasopharyngeal swab, oral swab, or other recommended collection procedures.
2. I authorize my test results to be disclosed to the county, state, CDC, Chinese Consulate General in San Francisco or to any other governmental entity that may be required by law or regulations.
3. I acknowledge that a positive test result indicates that I must self-isolate and/or wear a mask or face covering as directed in an effort to avoid infecting others.
4. I understand the testing unit is not acting as my medical provider, this testing does not replace treatment by my medical provider, and I assume complete and full responsibility to take appropriate action with regards to my test results. I agree I will seek medical advice, care and treatment from my medical provider if I have questions or concerns, or if my condition worsens.
5. I understand that, as with any medical test, there is the potential for a false positive or false negative test result. I understand that there are risks and benefits associated with undergoing a diagnostic test for COVID-19.
6. Authorization and Consent for Covid-19 scientific research: I voluntarily consent and authorize Apostle Inc, Apostle Diagnostics to use the testing results to conduct scientific research for the purposes of better understanding and consequently better controlling COVID-19. I acknowledge and understand that the COVID-19 diagnostic results will be de-identified in the Covid-19 scientific research.
Release to the fullest extent permitted by law, I hereby release, discharge and hold harmless to Apostle Inc, including, without limitation, any its respective officers, directors, employees, representatives and agents from any and all claims, liability, and damages, of whatever kind or nature, arising out of or in connection with any act or omission relating to my COVID-19 diagnostic test or the disclosure of my COVID-19 test results.
I understand that, all the timelines described in the service descriptions are strictly estimates only, and cannot be guaranteed. Actual timeline may vary significantly due to volume and technical reasons.
I, the undersigned, have read, understand, and agreed to the statements contained within this form. I have been informed about the purpose of the COVID-19 diagnostic test, procedures to be performed, potential risks and benefits, and associated costs. I have been provided an opportunity to ask questions before proceeding with a COVID-19 diagnostic test and I understand that if I do not wish to continue with the collection, testing, or analysis of a COVID-19 diagnostic test, I may decline to receive continued services. I have read the contents of this form in its entirety and voluntarily consent to undergo diagnostic testing for COVID-19.
